Type of Credential Certification: (Effective July 1, 2011, criminal background checks are required for all practitioners applying for an initial license. Any application received by the office with a postmark on or after July 1, 2011 shall submit to a national criminal history background check at the cost of the practitioner. Exam Administered NCETM, NCETMB, MBLEx or NCE from the American Medical Massage Association
Renewal Fee: $100 every 3 years
